Telehealth Consent

By using PepGenex Health services, you consent to receive telehealth services from licensed healthcare providers through Stealth Medical Group PA. Please read this consent carefully before proceeding.

1. Nature of Telehealth Services

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a licensed clinician and a patient who are not in the same physical location. The telehealth services provided through PepGenex Health are asynchronous — your intake form and medical history are reviewed by a licensed physician without a real-time video or phone interaction, unless your provider determines one is clinically necessary.

2. Participating Providers

Medical services are provided by Stealth Medical Group PA, an independent professional association of licensed physicians and nurse practitioners. Providers are licensed in the state(s) where they practice and where you, the patient, are located. PepGenex Health is a technology platform and is not a medical provider.

3. Your Rights

You have the right to:

• Withhold or withdraw consent for telehealth services at any time without affecting your right to future care or treatment.
• Inspect all information obtained and recorded during the telehealth interaction.
• Receive information about alternative care options if telehealth services are not clinically appropriate.
• Expect that all laws regarding patient confidentiality shall apply to telehealth services.

4. Potential Risks

As with any medical service, there are potential risks associated with telehealth. These include but are not limited to: technical failures that may interrupt or delay care; limitations in the physical examination compared to in-person evaluation; and the possibility that your provider may determine that telehealth is not appropriate for your condition and refer you to an in-person provider.

5. Privacy and Confidentiality

All telehealth services are subject to HIPAA (the Health Insurance Portability and Accountability Act). Your protected health information will not be shared without your explicit written consent except as required by law. PepGenex Health uses industry-standard encryption to protect data in transit and at rest.

6. Prescription Medications

If your provider determines that medication is clinically appropriate, your prescription will be fulfilled by an FDA-registered compounding pharmacy and shipped directly to you. All medications are compounded per a valid prescription and applicable regulatory guidelines. You acknowledge that you will follow your provider's dosing instructions and report any adverse effects promptly.

7. No Emergency Services

PepGenex Health is not an emergency medical service. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this platform for urgent or emergency medical needs.

8. Consent

By proceeding with your health intake or consultation through PepGenex Health, you acknowledge that you have read and understood this consent form and agree to its terms. You confirm that you are 18 years of age or older and are the person whose information is being submitted.
